Patients and families navigating a new diagnosis or changes in their health often find it helpful to know about the likely path ahead in order to make informed decisions to enable them to stay in control of their care over time.
Illness complexity and multimorbidity makes communicating about the future challenging. We suggest leaning into this uncertainty by focusing on what you do know instead of what you don’t know. Put simply, we know the path ahead is often unpredictable; it’s our job to communicate that to the people we care for in easy-to-understand terms to allow them to plan accordingly for their future.
